U.S. Customs Form: CBP Form 3347A – Declaration of Consignee When Entry is Made by an Agent

U.S. Customs and Border Protection Form 3347A is used to declare the true consignee for a shipment when the entry is made by an agent.  In many cases, a Customs Broker or other transportation agent may make the entry of goods on behalf an importer.  This form is used to declare the name of the true consignee (a fancy word for “importer” used in the industry).
