U.S. Customs Form: CBP Form 3461 – Entry/Immediate Delivery

U.S. Customs and Border Protection Form 3461 is the most commonly used Customs form.  The 3461 form is  required for all imports of goods for commercial re-sale.  Also known as a consumption entry, these imports are pretty straightforward and any Customs Broker should be able to file it for you in under 15 minutes unless there is something peculiar with your shipment.
