U.S. Customs Form: CBP Form 434A – North American Free Trade Agreement Certificate of Origin Continuation Sheet

U.S. Customs Form 434A is a continuation of the NAFTA Certificate of Origin form.  In the event an importer runs out of room on the original form, they can use this one to continue describing the imported merchandise to verify that they did indeed originate in a NAFTA country.
